Facial Feminization

A set of surgical and aesthetic procedures to soften features and harmonize the face, revealing your true essence.

Facial feminization is considered a group of surgical procedures derived from plastic and craniomaxillofacial surgery aimed at altering the masculine characteristics of a face. These techniques can be used in female patients (cisgender women) who have markedly masculine features or, more frequently, in transgender patients transitioning from male to female (MtF).

Diagnosis and surgical planning are based on a combination of skeletal assessments through X-rays, anthropometric measurements, CT scans, photographs, and computer programs. The patient's self-assessment and, especially, the surgeon's experience are very important to assist in surgical planning.

Techniques Used

Main Procedures

  • Frontoplasty: Reshaping of forehead bones and eye sockets.
  • Canthopexy: Elevation of the outer corner of the eyes.
  • Facial Contouring: Redefinition of the jaw and chin (mentum).
  • Chondroplasty: Reduction of the "Adam's Apple" (thyroid cartilage).

  • Lipofilling (fat grafting)

  • Blepharoplasty (eyelid surgery)

The Frontoplasty Technique

The surgery involves making an incision in the scalp area, which can be intracapillary (hidden within the hair) or along the hairline. It is important to note that whenever scalp advancement is necessary, the incision must be at the hairline.

Type I

Performed through selective bone reduction using rotary instruments and orbital reshaping. There is no removal of bone portions. Indicated for patients with thick frontal bone plate and little projection.

Type III

Involves the removal of a portion of the bone, reshaping and repositioning, followed by fixation with titanium miniplates and miniscrews. Allows for more expressive corrections of bone projection.

Outer Eye Corner

Facial feminization is the sum of modifications in small anatomical details. The intercanthal axis is straighter in men. Techniques to tilt the outer corner of the eye (canthopexy) can be applied to create a more feminine and "almond-shaped" look.

Hairline

The hairline height can be modified by scalp advancement surgery. Temporal recessions can be minimized by hair transplantation (FUE) or flap technique (FUT), softening the forehead shape.
